How to Enroll
1. Have required documents listed below
2. Fill out and print enrollment forms
3. Bring forms and documents to the building your child will attend

Required Documents for Enrollment
*Birth certificate or other acceptable proof of age
*Immunization Record
*Proof of residency-acceptable documents include: property tax statement, legal property description, utility   bill/agreement, rental agreement/receipt, or real estate contract. 
*Discipline from previous school(s) (Ozark R-VI schools will request this from the previous school(s) attended)
*Transcript of grades (high school only)
*Transfer grades if moving in the middle of a quarter or semester (for 7th – 12th grade only)
Name, address, fax and phone number of previous school
Picture ID of parent
Proof of guardianship/custody papers, if applicable
Child’s Social Security Card- for high school only

*items required by the district before your child will be able to start school
